Challenge

Advancing RPM to meet massive demand

Remote patient monitoring (RPM) and care have grown explosively since 2020, but the technology to support it is often fragmented, underpowered, or unavailable.

Nsight Health set out to leverage RPM to deliver preventative, value-based care proven to reduce hospitalizations, through ongoing monitoring and personalized health counseling. In just two years, under the leadership of two-time “Inc. 500” CEO Mike Ciullo, Nsight’s chronic care management and behavioral health intervention teams were serving hundreds of thousands of patients across the US, with large healthcare networks clamoring to sign on. But to scale their care service, Nsight needed a new, unified platform designed to support the entire remote care cycle, rather than the inefficient mashup of systems that were limiting its growth.

Process

Removing friction for faster remote care

Remote care requires the coordination of people, hardware, and software, and every connection point has the potential to slow care delivery. We worked with Nsight’s team to map the remote monitoring and care journey, identify pain points, and design a platform that gives providers the right information and tools to get each patient the help they need, when they need it: PRVNT.

Calling, centered

Nsight’s care model depends on connecting with patients by phone – to answer questions, discuss health plan progress, and check in about worrisome RPM reading. PRVNT embraces the phone’s central place in the user journey, fully integrating auto-dialing software with SOAP (for “subjective, objective, assessment, and plan”) notes, RPM data, and other essential patient information to let providers focus on patient care.

We compiled input from Nsight care providers and analyzed best-in-class EHR tools to design an integrated Patient Chart feature with SOAP notes, synced with the onboard calling software to let users access, review, and update critical patient data with no delay.

Customized filtering for every role

A range of stakeholders rely on PRVNT to perform their patient care roles, from Nsight’s own clinicians and administrative teams, to PCP, conditional specialist, or other in-person healthcare provider. Role-based permissions automatically curate and reconfigure the information each user sees, helping them prioritize tasks while protecting patient privacy.

PRVNT in patients’ pockets

The PRVNT mobile app keeps patients informed and engaged in their remote care programs, allowing RPM patients to monitor their paired device readings over time (from glucometers, spirometers, and more) and enabling them to ensure critical health data like allergies and medications is up-to-date.
